持续的躯体形式的疼痛障碍患者的防御机制及相关因素研究

摘要:

目的:对持续的躯体形式的疼痛障碍(以下简称持续疼痛障碍)患者的心理防御机制及相关因素进行研究。方法:持续疼痛障碍组(70例)和正常对照组(60例),入组时填写防御方式问卷(DSQ)、艾森克个性问卷(EPQ)、生活事件量表(LES)及社会支持评定量表(SSRS)。结果:①病人组在“不成熟防御机制”维度上的得分高于对照组,其中以抱怨、幻想、退缩、躯体化4个因子得分突出;在“成熟防御机制”的维度上,病人组得分明显低于对照组。②病人组神经质评分(N)显著高于对照组,而内-外倾评分低于对照组。③患者LES的总分及频次显著高于对照组,但社会支持分比对照组低。结论:生活事件、社会支持缺乏、个性缺陷和不适当的防御机制在持续疼痛障碍发病中起重要作用。

Objective:To explore psychological defense mechanism and its related factors in patients with persistent somatoform pain disorder.Methods:Both persistent pain disorder patients(n=70) and normal controls(n=60) were measured with DSQ,EPQ,LES,SSRS.Results:①The factor scores of immature defense mechanism were significantly higher and the factor scores of mature mechanism were signicantly lower in patients than in normal controls,especially in complaint,recoil,fantasy,and somatication factors;②The N scores of EPQ were significantly higher and the E scores of EPQ were significantly lower in patients than in normal controls;③Compared with the control group,patients had higher scores and more frequency in LES,but the scores of SSRS showed much lower.Conclusion:Life events,deficiency of social supporting,maladaptive personality and inadequacy defense mechanisms played important roles in the onset and development of persistent pain disorder.
